We have an immediate need for a Case Manager working out of our South Portland office and traveling throughout southern Maine. If you have a minimum of two (2) years experience in a Long Term Care or hospice environment, (previous management or supervisory nursing preferred), we welcome you to call us to find out more about this position. An Allegiance Case Manager plans, coordinates and delivers nursing care to hospice patients and their families according to individual needs: You will also manage and coordinate care plans from referral to discharge; perform pain assessments; facilitate interdisciplinary team meetings; apply the policies and procedures of the hospice facilities and Federal and State regulatory agencies; supervise home health aides; and maintain and update documentation.
