Positions with an "FTE" (full time equivalency) of .875 - 1.0 are eligible for full-time medical, dental and vision benefits. Positions with an "FTE" of .5 - .874 are eligible for part-time pro-rated medical, dental and vision benefits

The primary responsibility of the Community Nurse Coordinator is to support the strategy, development and ongoing implementation of Froedtert Hospital's Community Health Improvement programs. Preparing and monitoring a comprehensive Community Health Needs Assessment (CHNA) and Community Health Improvement Plan (CHIP) is a major responsibility of this position. Reporting to the Director, Community Engagement, this position works in close collaboration with Froedtert Health and Froedtert & the Medical College of Wisconsin stakeholders to improve the quality of life in the communities we serve through healthcare programs and services that are measurable, accessible and culturally appropriate; recognizing the greatest opportunity for impact is in Milwaukee's underserved, urban population. For professional nursing practice issues, the Community Nurse Coordinator reports to the Chief Nursing Officer. This job description is adapted from the ANA Nursing: Scope and Standards of Practice, 2nd Edition (2010). Other duties as assigned.

Eight plus years experience in public health and/or providing care for vulnerable populations is required. Experience in health education program design and delivery is required. Pediatric care and supervisory experience is preferred. Degree in Nursing (Baccalaureate or Graduate) is required. Master?s degree in Nursing is preferred. Requires current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact). Current WI driver?s license is required.

Other information:
Familiarity with public health data and analysis, as well as population health indicators is required. Excellent communication, interpersonal and customer service skills necessary to interact with senior hospital staff, clinicians, researchers and diverse patient populations. Ability to manage multiple projects and deadlines. Familiarity with budget, strategic and business plan development. Expertise in Microsoft Office products, including Word, Excel and PowerPoint is required. Proficiency in internet research necessary. Familiarity with database management desired. Must demonstrate commitment to healthcare information privacy as well as the ability to process confidential information in a sensitive manner.
