Dependable Home Health is currently seeking a compassionate, reliable Home Health LPN to work as our Intake Review Coordinator. The ideal candidate will possess 1 year of nursing experience and must be proficient with Face to Face visits, reading referrals, PDGM DX codes, and great phone communication with Physicians. This individual will be paid weekly and offered $1000 BONUS for full-time and $1500 BONUS for full-time Spanish-speaking LPNs.

Requirements of the Home Health LPN

Must have a current valid Arizona State Board of Nursing LPN License
1 year of nursing experience
Possess a current valid Arizona Class C Driver's license
Have reliable personal transportation
Have a current BLS Certification
Have, or be able to obtain, a TB clearance health card that meets federal/state requirements
Have, or be able to obtain, a satisfactory fingerprint clearance that meets federal/state requirements
Community/home health or medical/surgical experience is preferred
Strong new grads will be considered
Spanish speaking is a plus!
Responsibilities of the Home Health LPN

Complete the clients Face to Face following Agency, State, and Medicare policies and requirements
Have experience reading referrals
Follow the plan of care, provide treatments, and work collaboratively with the members of the team to help meet positive patient care outcomes
Experience with PDGM DX coding
Provides accurate and timely documentation
Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor.
Uses and/or prepares equipment and supplies effectively and efficiently adhering to aseptic technique. Participates in personal and professional growth and development.
Performs other duties as assigned by the RN Clinical manager
