We are a thriving and growing family practice located in SW Phoenix, AZ. We will consistently strive to provide accessible, high quality care within the local community. Our focus is on improving the health and well-being of all individuals by offering outpatient family healthcare for all ages.
Duties Include:
Provide primary clinical care of clients of all ages in accordance with standardized procedures. Clinical care includes the following:
Obtaining health history
Performing physical examination
Assessing and managing physical and psychosocial problems.
Developing a problem list, a diagnostic, therapeutic and teaching plan, which includes preventive health.
Managing emergency cases.
Patient education and counseling.
Furnish and adjust medications in the formulary in accordance with the law.
Order diagnostic studies such as laboratory and radiologic tests, as needed.
Refer patients for specialty evaluation and care as needed.
Maintain medical records of care given. Fills out all necessary forms.
Review patients records, including laboratory data, for abnormalities and completeness and is responsible for providing appropriate follow-up
Qualifications:
Must have a FNP degree from an accredited university or school.
Must have 2 + years experience as Nurse Practitioner.
Must be licensed in the state of AZ
Hours Of Operation:
Monday through Friday from 8:00 am to 5:00 pm
Salary 110,000 to 130,000 annually.
