1.0 FTE is Full-Time any position less than 1.0 FTE is Part-Time and .001 FTE is Optional Part-Time

Positions with an "FTE" (full time equivalency) of .8 - 1.0 are eligible for full-time medical dental and vision benefits. Positions with an "FTE" of .5 - .799 are eligible for part-time pro-rated medical dental and vision benefits

Shift: 1st. Monday-Friday 8am-4:30pm. 1 weekend per quarter.


The Case Manager RN/Educator is a practitioner and educator. As a practitioner the Case Manager RN/Educator is responsible for coordinating care and services of selected patient populations through the acute care episode and across the continuum to support an effective discharge plan. As an Educator the Case Manager RN/Educator collaborates with Organizational Learning participating in the design implementation and evaluation of orientation continuing education and in-service education relevant to Care Management staff patient population(s) practice setting regulatory compliance and patient and family education. The Educator uses tools theories skills and knowledge to maintain Care Management standards of practice and to improve the care management practice of learners. The Case Manager RN/Educator is available for week days and weekend staff based on individual educational needs. The Case Management RN/Educator serves as a role model and has a responsibility to foster a positive attitude about the benefits of change and opportunities of life-long learning. The Case Management RN/Educator job description has been adapted from the ANA Nursing Professional Development: Scope and Standards of Practice 3rd Edition (2016) the ANA Nursing: Scope and Standards of Practice 3rd Edition (2015) and the American Case Management Association Standards of Practice and Scope of Services (2013). Other duties as assigned.


A minimum of 3 years of applicable clinical practice experience as a licensed registered nurse and at least 2 years experience in the case management field including utilization review transitional planning outcomes management readmission reduction and discharge planning assessment is required. The ability to effectively assess staff members' educational needs design programs to meet identified needs and to oversee instruction for staff members. Degree in Nursing (Baccalaureate or Graduate) is required. Professional knowledge of nursing theory educational theories and principles to assist in the process of providing education to a professional staff is required. Accredited Case Manager Certification (ACM) or Certified Case Manager (CM) certification required within 2 years. Requires current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact).

Other information:

General knowledge of payor industry (Medicare Medicaid and General knowledge of payor industry (Medicare Medicaid and Commercial insurance regulations) resource management and evidenced based clinical practice is essential. Theoretical knowledge of the nursing and case management processes. Analytical skills necessary to assess patient's discharge needs and develop solutions to problems concerning patients' mental status or physical well-being to ensure optimal patient outcomes. Advanced communication and interpersonal skills with all levels of internal and external customers including but not limited to medical staff patients and families clinical personnel support and technical staff outside agencies and members of the community. Other skills needed include excellent written/verbal communication skills critical thinking creative problem solving proficient organization and planning. Basic computer skills. Must be self-directed with ability to tolerate frequent interruptions and a demanding workload.
