Must be enrolled in the LA Medicare/Medicaid insurance program
DOT certified a plus
LA License & DEA
Will reimburse for mileage, but will not cover any other travel/lodging expenses
Facility: Open locations in Crowley, Jennings, Lake Charles, and Moss Bluff
Schedule: Start ASAP
Mon-Fri 8am-7pm
Sat-Sun 8am-4pm
3 days per week
Assignment: Urgent Care
Average 18-20 patients/day
Treat all ages
Prevention & Wellness Checks as well as occupation health services
